Nonclinical Roles at the Care Connection Center

The VITAS Care Connection Center (CCC) offers multiple administrative roles, positions critical to keeping the CCC operating smoothly.

These roles require equal parts customer service and administrative duties to meet the needs of our referral partners, patients and families, facility partners, and all internal and external customers.

For all positions, an in-depth five-week training program gets team members off to a good start, and supportive leaders help you find possibilities for growth.

Intake Team

On the intake team, admissions coordinators work with referral partners, program admissions teams, and patients and families throughout the admissions process, from referral to enrollment decision. The intake team operates 24 hours a day, 365 days a year, to ensure patients can initiate access to hospice care when they need it.

Telecare Team

On the telecare team, patient care coordinators operate during the hours of 4 pm to 8 am on weekdays and around the clock on weekends to triage calls, ensuring a 24-hour continuum of care for all VITAS patients and families.

Continuous Care After-Hours

On the continuous care after-hours team, continuous care coordinators directly serve continuous care patients through direct collaboration with program patient care teams. These team members utilize all tools and training provided to ensure clinical coverage at the bedside for patients requiring a higher level of care.

Pharmacy Infusion

Pharmacy infusion technicians play a critical role in ensuring patients receive their infusions in a timely manner. They coordinate the ordering, fulfillment, and delivery of infusions, working with clinicians and field operations staff to ensure a seamless process.


Administrative team positions include various leadership and supporting CCC roles, including analysts, schedulers, and more.
