At the end of life, some medication changes are unavoidable. Our physicians are available to assist with medication changes at any time–day or evening, weekends and holidays. VITAS maintains an open formulary to ensure that patients can continue the medications associated with their terminal diagnosis that help manage pain and improve their quality of life.

Open Formulary With 24/7 Clinical Support

With VITAS, your residents have access to any medication associated with their terminal diagnosis–and your staff always has support. VITAS clinicians are always available to answer questions and educate your staff on dosages and pill box management. When a VITAS patient in your facility becomes acutely symptomatic or noncompliant, VITAS can send a clinician at any time–day, evening, weekend or holiday–to provide assistance and administer medication.

Physician-Supported Medication Changes

Your resident may be hesitant to give up the medications prescribed as part of their current care plan. They may fear a tumultuous transition from one medicine to the next or worry that their symptoms won’t be properly managed.

With guidance from VITAS medical directors and hospice physicians, your residents’ wishes and goals at the end of life will be incorporated into a care plan that meets their clinical, emotional and spiritual needs–including the continuation of any prescriptions currently providing symptom relief.

We partner with your residents’ specialists and primary care physicians to facilitate hospice access and provide patients with the smoothest transition possible. Because of our open formulary, we can assure prospective patients that while adjustments will occur as part of their shift from curative care to hospice, they need not give up all treatments to receive our care.

Non-Pharmacologic Treatment

To complement our open formulary, VITAS integrates non-pharmacologic approaches to symptom relief and improved quality of life, including but not limited to:

  • Respiratory therapy
  • Music therapy
  • Paw Pals® pet visits
  • Massage therapy

Home Medical Equipment and Supplies

VITAS maintains its own home medical equipment division, allowing for seamless transitions to hospice and quick delivery of important equipment and other necessities, including but not limited to:

  • ADL assist devices
  • Oxygen and delivery equipment
  • Nebulizers
  • CPAP and BiPAP
  • Walkers and canes
  • Incontinence and wound care supplies

Expert Dementia/Alzheimer’s Care

VITAS is prepared to meet the unique needs of patients who have Alzheimer’s/dementia and who wish to remain where they live. Our interdisciplinary team develops a care plan with the advice and consent of your resident’s neurologist or other physician; a team manager coordinates the flow of information between our personnel and your staff.

VITAS is prepared to administer antipsychotics when a patient’s condition requires them; our proprietary protocol typically reduces patient reliance on antipsychotics.
