Case Study:
Patient with Advanced Heart Failure

The Challenge
AM, 86-year-old female, compliant with cardiologist and PCP follow-ups. In the last 6 months, AM has had increased visits to office due to intensifying symptom burden. Hospitalized with fatigue, dyspnea and a recent fall.
Goal
To stop repeated hospitalizations and improve patient’s quality of life.
Solution

After two days in the hospital, discharge care options were reviewed with AM’s daughter, including hospice, skilled nursing facility and home health. To honor AM’s wish to remain at home, AM was referred to hospice.

The daughter selected VITAS for its American Heart Association certification, ability to continue guideline-directed medical therapy and symptom management. VITAS coordinated a same-day discharge to home, including delivery of medications, supplies, home medical equipment and initiation of therapy services.

An entire team of clinicians with specialized cardiac training worked together to manage AM’s symptoms and make her comfortable at home. A VITAS physician, nurse, and aide visited regularly to manage her SOB and make her comfortable.

The same team collaborated with her cardiologist and provided AM’s daughter and family with education on how to take care of her at home. All of the equipment and medications AM needed were delivered to her at home by VITAS, including cardiac medications, inhalers, portable oxygen, hospital bed and a portable toilet.

A social worker helped with emotional issues and discussed AM and her daughter’s goals for her care, while a chaplain supported AM and her family with spiritual issues.

A grief specialist was available for up to 13 months after death to support AM’s family.

Result
The patient remained at home, avoided further disruptive care transitions and died peacefully with loved ones present.
