VITAS Is Proud to Serve the Ocean Reef® Community

Through our partnership with Baptist Health South Florida, VITAS provides compassionate hospice care and educational support to Ocean Reef^® patients and families who face the physical and psychosocial challenges of advanced illness.

Inspired by our guiding value of putting patients and their families first, our hospice teams provide a range of services to help residents, caregivers and Ocean Reef^® staff understand and address end-of-life challenges with confidence, competence and compassion:

• Goals-of-care conversations to help patients and families understand their diagnosis, prognosis and options for care
• Chaplain support to explore spiritual beliefs and end-of-life care
• Social worker support for maintaining hope while living with advanced illness
• Advance care planning, including personalized care plans based on patient/family wishes, values and preferences
• Completion of advance directives and legal documents (e.g., living will, medical power of attorney, DNR, Five Wishes)
• On-site community education and access to the VITAS library on dozens of topics in multiple languages (caregiving, diseases, safety, symptom management, medications, coping strategies and more)
• Grief and bereavement support for 13 months
