About This Webinar for Physicians and Healthcare Professionals
A case study of a 66-year-old patient provides the backdrop for two diagnostic scenarios—sepsis and post-sepsis syndrome, both of which are leading contributors to in-hospital mortality and complex post-acute care.
Attendees will explore the natural history of sepsis, indicators of poor prognosis for both conditions, the benefits of goals-of-care conversations and early identification of hospice eligibility among patients with sepsis, the value of timely referrals to end-of-life care, and a hospice care model for sepsis.
This webinar is offered for both CE and CME credit:
- CE credit is available to nurses (RN, LPN, LVN; in California Registered Nurses only), social workers and certified case managers, as well as licensed nursing home administrators in the states of Florida and Illinois.
