Medical Director - Orange County
Chalat Rajaram is a Medical Director for VITAS Healthcare, the nation’s leading provider of end-of-life care. He is responsible for overseeing the medical care and treatment of VITAS patients in their homes and in nursing homes and assisted living communities in Orange County, California. He also consults with patients’ attending physicians, supervises VITAS team physicians, guides staff and volunteers, and serves as a resource to practicing physicians and other healthcare professionals on the benefits of hospice and palliative care for patients and their families.
Dr. Rajaram joined VITAS in 2006 as part of an interdisciplinary patient care team, providing pain and symptom management and social and emotional support to VITAS patients and their families. He was promoted to Medical Director in 2008. In addition, Dr. Rajaram is a post-acute-care specialist and medical director of skilled nursing facilities in Orange County. Earlier in his career, he was Medical Director for Sutter Health in Sacramento, where he oversaw the Program of All-Inclusive Care for the Elderly (PACE), which has been recognized as a model of comprehensive care for the frail elderly population.
Board certified in Internal Medicine, Dr. Rajaram earned his pre-medical education at St. Joseph’s College and his Medical Degree from Calicut Medical College, both in Calicut, India. He completed an internship at Harlem Hospital Center in New York City and a residency at Maricopa Medical Center in Phoenix, Arizona.
